Taxon: The scientific name of the organism that you saw. If you can identify down to the species level (e.g. Myotis Lucifugus), that's great. If not, indicate the lowest rank that your confident in, (e.g. Chiropetera).

Common name: What you saw, in common English usage.

Request Identification (Y/N): Put a Y if you would like an expert to look at a picture that you took. You can do this if you have no idea what you saw, or if you just want to be sure.

Notes/observation: Anything you want to add to provide context to your observation.

Number: An estimate of how many you saw. The default is 1. Use "0" to indicate that something was found to be absent. Please give just a number, and other description can go in the comments box.

Date: Date of the observation. Use MM/DD/YYYY or MM-DD-YYYY.

City/town:

State/Province:

Country:

Latitude: Please use decimal degrees. -90 < Latitude < 90.

Longitude: Please use decimal degrees. -180 < Longitude < 180.

Observer: Who made the observation?

Blogger: Who is reporting the observation?

Blog URL: Where can we find your blog posts?

Image URL: If you've posted an image of this observation on-line, put the URL here.
